Tax Exemption: Big Dee's Tack and Vet Supply must have a valid state tax exemption form on file for your farm/business. If you are tax exempt, please fill out the forms from your state and email to karenh@bigdweb.com, fax to 330-422-1175 or mail to 9440 State Route 14 Streetsboro, Ohio 44241

** If you ship to additional states, you will also need to complete the appropriate tax exemption for select states that you have orders shipped to.

Alabama - Big Dee's Tack and Vet Supplies has collected the simplified sellers use tax on taxable transactions delivered into Alabama and the tax will be remitted on the customer's behalf to the Alabama Department of Revenue.

Arizona - Must fill in completely and sign Arizona Form 5000. Click here for form

Georgia - Must fill in and sign Georgia Sales Tax Certificates of Exemption ST-5. Click here for form

Indiana - Must fill in and sign Indiana form ST-105 and provide taxpayer id number. Click here for form

Iowa - Must fill in and sign Iowa state tax exemption certificate Click here for form

Massachusetts - Must fill in and sign Massachusetts blank exemption form Click here for form

Michigan - Must complete and sign Michigan Sales Tax and Use Tax Certificate of Exemption. Click here for form

Minnesota - Must complete and sign Minnesota form ST3. Click here for form

Missouri - Must complete and sign Missouri form 149. Click here for form
